新生儿胆红素脑病诊断中磁共振成像在临床价值探讨

摘要

目的 对临床诊断新生儿胆红素脑病过程中磁共振成像(MRI)的应用价值进行探讨。方法 选择90例新生儿胆红素脑病患者开展研究(2021年4月至2022年5月),根据患儿血清胆红素水平划分为轻症(n=30)、中症(n=30)和重症(n=30)等三组,患儿入院检查使用的都是颅脑磁共振,针对三组患儿检查过程中左右侧苍白球T1WI信号强度以及T2WI信号强度进行观察,同时对比两种信号比率。结果 经MRI扫描检查发现,在左右两侧苍白球T1WI信号方面三组患儿无明显差异,P>0.05,随着病情进展,上述患儿左右两侧苍白球T1WI信号均出现明显提升,其中重症组信号强度最大,轻症组信号强度最弱,差异显著,P<0.05,而左右两侧苍白球T2WI信号方面三组患儿组内无明显差异,P>0.05;而且与信号检出率相比较,三组患儿T1WI高信号检出率明显更高,而且该检出率与病症严重程度存在正相关关系,差异显著,P<0.05。T2WI的信号检出率无明显差异,P>0.05。结论 在临床诊断新生儿胆红素脑病的过程中左右苍白球T1WI信号枪都可以作为一种有效检测指标,其体现出了高分辨率、检测可靠性等一些特征,具有极大临床应用价值。

Abstract

Objective To investigate the value of magnetic resonance imaging (MRI) in the clinical diagnosis of neonatal bilirubin encephalopathy.
Methods 90 cases of neonatal bilirubin encephalopathy were selected for the study (April 2021 to May 2022). According to the serum bilirubin level of the children, they were divided into three groups: mild (n=30), moderate (n=30) and severe (n=30). The patients were all examined with brain MRI. The T1WI signal intensity and T2WI signal intensity of the left and right globus pallidus were observed during the examination of the three groups, and the two signal ratios were compared.
Results MRI scanning showed that there was no significant difference in T1WI signals of the left and right globus pallidus between the three groups (P>0.05). With the progress of the disease, T1WI signals of the left and right globus pallidus of the above children were significantly enhanced, and the signal intensity of the severe group was the largest, while that of the mild group was the weakest, with significant difference (P<0.05), while there was no significant difference in T2WI signals of the left and right globus pallidus between the three groups (P>0.05); Compared with the signal relief rate, the detection rate of high signal on T1WI in the three groups was significantly higher, and the detection rate was positively correlated with the severity of the disease, with significant difference (P<0.05). The signal detection rate of T2WI had no significant difference (P>0.05).
Conclusion   In the clinical diagnosis of neonatal bilirubin encephalopathy, both left and right globus pallidus T1WI signal guns can be used as an effective detection indicator, which reflects some characteristics such as high resolution and detection reliability, and has great clinical application value.
